Survey Fields

The table below lists the fields of the Survey form and provides utilization guidelines for each field.

Field Label Utilization Guideline
Picture

The Picture field is used to add a picture or logo of the survey.

Click on the picture to enlarge it or to change it.

Name

The Name field is used to enter the name of the survey.

Disabled

The Disabled box is checked when the survey may no longer be used to rate a service.

Introduction

The Introduction field is used to give some general guidelines to the user (i.e. average response time). This introduction is shown before the first question.

This field allows basic text formatting.

Question

The Question field is used to ask a question about the service to the person who is rating the service.

Guidance

The Guidance field is used to give additional information about the question.

This field allows basic text formatting.

Type

The Type field is used to indicate whether the question is a star rating question or a text question. Only star rating questions are used in calculating the service rating. If a survey does not contain at least one star rating question, the responses to this survey will appear in the ‘Partial’ tab of the ‘Survey responses’ section of the Analytics console.

Weight

The Weight field is used to define the weight of a star rating question in the calculation of the service rating. The relative weight of a question is automatically shown as a percentage (%) next to this field.

For example, if a survey has two star rating questions with weights 100 and 200 (or 50 and 100), the relative weights used in the calculation of the service rating are 33% and 67%

Disabled

The Disabled box is checked when the question may no longer be asked in this survey.

Completion

The Completion field is used to thank or give feedback to the user after the final question was answered.

This field allows basic text formatting.
